What You'll Do:
- Monitor client conditions, perform focused assessments, and report findings and changes to the RN in a timely manner
- Ensure unlicensed staff provide high-quality, timely care and address performance or training needs with the RN
- Implement and document physician orders, assist with care plan updates, and ensure compliance with agency policies and state regulations
- Administer medications accurately, maintain proper documentation, and respond promptly to emergencies or incidents
- Support client admissions, transfers, and discharges while maintaining open communication with clients, families, and interdisciplinary teams
What You'll Need:
- Active MN LPN License
- Minimum of 1 year of experience in home health, med-surg or acute care preferred
- Prior Supervisory experience preferred
